LiveFORM: Form builder

To build your auto-filling web form that uses LiveFORM^TM technology, do these 4 simple steps:

Unzip files to one directory on your web server.
Select fields from the following form that you would like to see in your form. Do not enter any information in the fields they are only meant to indicate how the form will look eventually.
Click "Build Form". The form will be created and the browser will prompt you to save the file to a directory. Store the file in the same directory where you unzipped files in the first step.
And just in case ..... if you can not make it work then write to us and tell us which of these 4 steps failed for you!
